Our service includes:
Preparation of your SNF Medicare cost reports
as required by HCFA and your fiscal intermediary. Upon timely receipt of
requested information cost reports will be completed prior to their due
date. Each cost report will be completed in compliance with HCFA HIM-15
and PPS rules and regulations. You will receive 2 copies of each facility's
completed SNF Medicare cost report with supporting work-papers, along with
a diskette containing the electronic cost report for each facility for submission
to you fiscal intermediary. You will also receive a completed HCFA-339 questionnaire.
This service also includes answering any desk review questions as they relate
to cost report preparation. An invoice will be sent to the contact person
after all cost reports are completed. |

| Upon receipt of this order form, we will contact you to establish
communication, assess your needs, due dates, any special circumstances.
We will determine a fee for cost report preparation and prepare a cost report
preparation engagement bid for your review and approval. |
