Instructions on how to Download and Save an Excel Document to your Computer

PC: Click the link of the document shown above, click Save or Save this program to disk, and choose a location on your computer to save the form.

Macintosh: Click the link of the document shown above and the form will download to your desktop.

You will now be able to open the Excel file in Microsoft Excel (full version) and enter values in the form, save it as a new file and print out or email for submission.

FREE Microsoft Excel Software

You will need to have Microsoft Excel software installed on your computer in order to view, navigate and print Excel documents. If you currently do not have Excel software installed, then please download and install the FREE Microsoft Excel Viewer.
